Post COVID-19 Vaccine and Autoimmune Hepatitis

Background and Aim: The emergence of the COVID-19 pandemic led to development widespread use vaccines control disease. While these are generally safe, there have been rare instances vaccine-related adverse effects. In this retrospective review, we present two cases severe acute liver injury following Pfizer vaccination. Methods: We reviewed patients presenting with Covid-19 vaccine-induced in our database. Laboratory data biopsy findings were reviewed, including chronic disease evaluation. Results: Patient 1, a 67-year-old Hispanic female, experienced significantly elevated enzymes bilirubin levels after receiving four doses vaccine. Liver revealed Grade 4 Stage 1 lymphoplasmacytic infiltrate interface hepatitis, consistent autoimmune hepatitis patient was initiated on Prednisone, tapering down every 10 days. Upon completion prednisone therapy, she started Cellcept. She responded well treatment has maintained immunosuppressants. Chronic workup ruled out other underlying diseases. 2, 49-year-old Asian developed abnormal function tests single dose Subsequent evaluation indicated bridging fibrosis, confirmed by biopsy. Treatment azathioprine resulted favorable biochemical response. Conclusion: incidence is rare, but clinicians should be aware potential effect. benefits outweigh risks injury. It remains unclear if vaccine triggers an response or leads de novo hepatitis. Further research greater sample necessary understand mechanism identify risk factors. Additionally, determining optimal duration immunomodulatory crucial. Despite cases, overall safety efficacy support their continued combating pandemic.
